Characteristics of Brain-Muscle Coupling and Computer Signal Processing in the Treatment of Post-Stroke Hemiplegia by Double-Edged Needle Knife Stimulation of Periosteum

Background: Post-stroke hemiplegia is a relatively severe disease that can cause many long-term health problems and is also very expensive for the patient and society. Methods: A total of 68 patients with post-stroke hemiplegia were randomly divided into a treatment group (n=34) and a control group (n=34) for double-edged needle knife periosteal stimulation combined with conventional rehabilitation and conventional rehabilitation only, respectively. Results: The patients were included in the comprehensive assessment of Brunnstrom's staging (4.06 ± 0.63 vs. 3.22 ± 0.69, p <0.01), manual muscle testing (MMT) score (3.79 ± 0.56 vs. 2.66 ± 0.61, p <0.01), Glasgow Coma Scale (GCS) score (14.70 ± 0.45 vs. 14.62 ± 0.48, p> 0.05), and Mini-Mental State Examination (MMSE) score (26.83 ± 2.21 vs. 24.27 ± 2.54, p <0.01).The treated group had significantly higher Fugl-Meyer Assessment (FMA) scores (42.6±8.3 vs. 28.4±7.1, p<0.001) and Modified Barthel Index (MBI) scores (72.5±12.3 vs. 55.8±11.6, p<0.001) than the control group. EEG-sEMG coherence analysis showed increased beta-band (15-30 Hz) coupling in the treatment group (coherence value: 0.68±0.12 vs. 0.45±0.15, p<0.01). Conclusion: Double-edged needle knife stimulation of the periosteum significantly increases brain-muscle coupling in post-stroke hemiplegia.
